- A contract agreement, in its entirety (7 pages total) — see icon below
- Current certificate of insurance naming Logistics One Brokerage, Inc. as certificate holder including:
- $1,000,000 general liability insurance
- $1,000,000 automobile liability insurance
- Worker's compensation and employer's liability insurance
- Cargo insurance of $100,000 per vehicle
- A completed W-9 form — see icon below
- A copy of your Operating Authority & Surety Bond
